   AAASLP CLIENT GRANT APPLICATION            

    Purpose of the Award
      To assist a speech-language impaired client with expenses including a speech-language      evaluation, therapy, consultations, ENT appointments, augmentative devices or other      related services.

      The amount of the award is $500.00.
    
    Selection Procedures
       1. The application deadline is April 12, 2010. Applications may be submitted at the           April meeting of the AAASLP or may be mailed to:

                                P.O. Box 50126
                                Austin, TX 78763.
      2. AAASLP members may submit as many applications as needed.
       3. All applications will be reviewed by the awards committee. Selection of the recipient
          will include, but is not limited to the severity of speech-language impairment, financial           need, and previous treatment.
       4. The grant will be presented to the selected recipient in May.
