LifeKit is easy to make. It consists of a blank form for basic medical information, a container and stickers. A display box is helpful for making the LifeKit accessable. Provided here are templates, images and instructions to get started.

Blank LifeKit Personal Information sheet. Four forms are contained on one sheet.

LifeKit Personal Information Instruction sheet. Four sets of instructions are contained on one sheet. These should go on the back of the LifeKit Personal Information form to remind people how to use LifeKit when they bring it home.

Sheet of stickers image. Download and print on 8 1/2 by 11 inch sheets of stickers. You may need to download the image and then print using a graphics tool. Browsers tend to add margins and resize the image so that it does not fit correctly on a sheet of blank stickers.

Steps:

  1. Construct a display box.
  2. Print the medical information sheet and instruction sheet. Make double sided copies with the the medical information on the front and the instructions on the back. Cut sheet into four forms.
  3. Collect empty film containers.
  4. Print enough stickers for both container labels and door stickers. Adhere stickers on the outside of containers as labels.
  5. Fold medical information form to fit in container. Place a form and a door sticker inside of the container.
  6. Place finished containers in display box. You may want to place extra blank medical information forms with the display box also.
That's it! But don't stop there. Keeping the display box stocked is necessary.
